Charlyn Slade Brings Deep Industry Knowledge, Focus on Coordinated Care
Charlyn Slade, former vice president of APP Advisors, the consulting arm of Advocate Physician Partners (APP), has joined the Prepared Health advisory board.
Located in Rolling Meadows, Illinois, APP is a care management collaboration with Advocate Health Care, and a leader in population health management. Under Slade’s direction, APP Advisors was created to support healthcare systems across the nation in the transformation to value-based care.
She served as its vice president for eight years before retiring at the end of 2018.
During her tenure, APP Advisors successfully replicated APP’s “secret sauce” of clinical integration to help healthcare systems: improve quality, care coordination and efficiency; negotiate contracts to deliver better value for payers and consumers; and deliver significant rewards for top- performing physicians. APP Advisors worked with 15 clients from 12 states to develop and implement clinically integrated networks, with more than 10,000 participating physicians across the nation.
“Our goal was to teach other organizations to be as successful as Advocate Health Care in improving patient care,” says Slade. “Our approach to population health through clinical integration and value-based contracting is nationally recognized.”
A nurse by training, Slade began her career in the high-risk nursery at Lutheran General Hospital in Park Ridge, Illinois. In 22 years at the hospital, she held a variety of senior administrative roles focusing on women’s and children’s services.
She also helped launch CarePages Inc. in 2001, an early platform that built websites for patients, caregivers and families to build personal support networks.
“I’ve always been involved with extending care outside of the four walls of the hospital,” says Slade. “This fits very well into what we’re doing at Prepared Health–keeping patients and families at the forefront to make the best care decisions.”
