AI Helps Jefferson “Manage By Exception”
Jefferson Health in Philadelphia has remotely extended care to nearly 1,900 Covid-positive patients, and managed more than 24,000 “digital dialogues” to track symptoms and recovery, and give people a sense of connection with their care teams. Using AI technology, patients who require immediate attention are automatically flagged for direct follow up and further assessment.
Jefferson has activated our new PAC Bed Availability solution to automatically identify the number of Covid-positive and Covid-negative beds in their SNF network. Real-time access to this data helps case management teams ensure timely discharges and appropriate placements.
In addition, the hospital has identified new opportunities to extend care to more patients outside of the hospital, including patients with complex conditions such as Congestive Heart Failure and Chronic Obstructive Pulmonary Disease (COPD).

Since the start of the pandemic, BAYADA Home Health Care found that proactive Covid safety planning helped them improve client acquisition and staff retention.
